If you would like to schedule an appointment to be covered by your insurance company, please submit one of our new insurance client forms below. If you have any questions or concerns, get in contact with us and we will be happy to help!

Please note, an insurance verification is required before booking. We encourage you to submit your insurance information. Please allow up to one week for verifications. Our biller will contact you via email with benefit information and instructions to book.

A form a payment on file is required for all insurance clients, kept securely encrypted in Square.
